"Why are Police Record Checks required"?

Simply put, in order to meet the Volunteer Screening rules imposed by the OSA.

The OSA initiated a project requiring that organisations scrutinize any volunteers who come in contact with children. All volunteers who are working with any soccer team are obligated to comply to the Volunteer Screening standards. The Club has been very proactive with this initiative by implementing this requirement at both the Competitive and Recreational levels.

Do I have to take the Police Record Check to the Police Station?

No. Please bring it in to the GSC office. If you take it in yourself, you will be asked to pay a fee. As a volunteer organization we can submit batches of forms without being charged any fees.

What is the "Name of the Police Authorized Agency on the Consent for Police Record Check Form"?

This is the name of the organization for which the check is being done. In our case, this is "Goulbourn Soccer Club".

Who fills in the "Signature of witness on the Consent for Police Record Check Form"?

Find someone to sign this who isn't a relation to your family. This could be a friend, fellow volunteer, or a member of the club exectutive.

What do I fill in the "Reason for Consent section on the Form 1-Consent for Criminal Record Check Form"?

Description: "Youth Soccer Coach, Youth Soccer Assistant Coach, Youth Soccer Trainer, Youth Soccer Manager"

Organization: "Goulbourn Soccer Club"

Provide Age Details: Insert the Age Group and Gender of the children you will be coaching. i.e. for U14 put 13 and 14 year old Girls/Boy?
