GLSL Referee Match Report Procedures
Match Reports ("game sheets") are required for every GLSL League game.At the start of the game collect a match report from each coach.
Check the form to make sure the identifying information (game date, number, team information) is complete, that all players are listed by name and number, and that there are notes for any player playing up, playing across, or serving a suspension. The coaches do not need to sign the reports at this time. Absolutely do not start the game, under any condition, until you have both reports!! Delay the start of the game, if required. Note that the match reports should indicate team colour, and whether they are home/away. If the coach has no match report, give them a piece of blank paper and ask for each players full name, jersey number, date of birth and signature as a replacement AND remember to note the reason for the game start delay on all Match Reports.'
At the end of the game, stand near the centre of the field, off to the side during the team handshake, and while over-seeing the handshake complete the Match Reports:
- Note the score on each Match Report (make sure each report has the same score!)
- Make a brief note on any situations that should be brought to the attention of the Head Referee, the Club, or the League. Briefly note the name and number of any individual receiving a yellow or red card, as well as any incident needing a Special Incident report. Make the same notes on each Match Report and for yourself (separate paper) so you can complete your discipline report(s) later.
- Sign each Match Report.
- After the handshake, each coach will verify the score, read your notes, and sign each report. If a coach declines to sign the report, note that fact on the Match Report and continue the end-game process.
- Hand the completed/signed Match Report back to all coaches (Almonte, Carleton Place, Munster, Richmond, Stittsville). It is the coach's responsibility to hand in the Match Reports.
If you issued a Yellow Card, a Red Card, or have a situation worthy of a Special Incident report, you MUST fill-in and deliver the correct discipline or incident report within 48 hours of that game's end to the Goulbourn Soccer Office. A referee who fails to perform this task is subject to discipline themselves by the district or provincial soccer authority.

Caution Report: Download, edit (type in all information), print, and sign the document. Mail to the Goulbourn Soccer Office within 48 hours of the end of the match.
Dismissal Report: Download, edit (type in all information), print, and sign the document. Mail to the Goulbourn Soccer Office within 48 hours of the end of the match.
Special Incident Report: Download, edit (type in all information), print, and sign the document. Mail to the Goulbourn Soccer Office within 24 hours of the end of the match.
