Required Identification

IDENTIFICATION

CLEAR copies of TWO pieces of proper identification MUST accompany both consent forms. BOTH pieces of identification MUST CONFIRM THE PERSONS NAME AND DATE OF BIRTH. If the date of birth is on the back of the card a photocopy of the back as well as the front much be taken.

One piece of identification MUST also contain a PHOTO ID of the person.

Both pieces of identification will ensure that the check is being performed on the correct spelling of the name and date of birth.

Identification which can NOT be used: Health Card

The following is a list of identification, which provide BOTH THE NAME AND DATE OF BIRTH of an individual:
  • Birth Certificate
  • Baptismal Certificate
  • Hunting/Fishing Licence
  • Outdoors Card
  • Indian Status Card
  • Hospital Cards
  • Canadian Blood Donor Card
  • Immigration Papers
  • Canadian Citizenship Card
The following is a list of identification which provide PHOTO ID:
  • Driver's Licence
  • Government/Military Employment Card
  • Passport
  • Age of Majority Card
  • Government Employment Card
  • Canadian Citizenship Card
  • Indian Status Card
  • International Student Card
  • Permanent Resident Card

The above is a suggested list of identification that is acceptable, however any identification bearing the required information will be allowed.

Failure to provide proper identification will result in the return of the application, unprocessed. Due to the sensitive nature of the information being searched, NO EXCEPTIONS will be made.
